Caswell County Home Health Agency believes that patients recover better in their own homes.  The mission of the agency is to provide quality home care while unnderstanding the unique needs of each client.

Home Health Services are provided to those persons who are homebound, under the care of a physican, and in need of a skilled service.  Anyone may call and request the services.  A nurse will evaluate teh referral and contact the patient's doctor for orders.

Home Health Services can be paid for in a number of ways including: Medicare/Medicaid, Private Insurance, VA/CHAMPUS, and Workers' Compensation.  A sliding scale fee schedule is available for patients who qualify.

SKILLED NURSINGPHYSICAL THERAPY
° On call 24 hours a day, 7 days a week° Set up exercise programs

° Formation of diet plan and menus

The Community Alternative Program (CAP) allows Medicaid recipients with disabilities to receive the services necessary to allow them to stay at home and avoid or delay admission to the long-term care facility.

CAP provides (from the agency of choice):

  • Medical Services
  • Supplies (diapers, Ensure, etc.)
  • Durable medical equipment (beds, wheelchairs, etc.) as needed
  • Unlimited prescriptions with a minimal co-pay
